About the Position:

The Com Hab Supervisor is responsible for maintaining appropriate records. Perform monthly individual visits and follow-up on recommendations. The Supervisor will be responsible for designing program components that maintain high expectations, promote social and personal growth, and cultivate a caring, engaging community.

This is accomplished by providing an array of individual and family supports through a dedicated team of professionals and support staff.

Responsibilities:

  • Perform assessments in order to determine the individual's goals.
  • Supervise Com-Hab staff and handle all sensitive issues.
  • Responsible for contacting parents with regard to individual's progress.
  • Tracking daily attendance.
  • Communicating program information with families and Com-Hab staff on a regular basis as well as the Director of the Department.
  • Ensure that the Director of the Department is notified immediately on all issues arising from the operations of the program.
  • Schedule individual's visits a week in advanced and post them in the weekly schedule book.
  • Obtain questionnaire surveys from individual's families on a yearly basis.
  • Compile data on program for individual's progress and billing purposes (daily/weekly notes).
  • Conduct monthly visits to the individual's residence to ensure that the staff is performing their responsibilities in line with the goals established.
  • Handle other related duties as assigned by the Director
  • Participate in regular scheduled meetings as assigned by the Director
  • Train new/existing employees in writing goal sheets.
  • Conduct investigations of incidents reportable/non-reportable.
  • Complete individual's documentation service (Community-Hab Plans on a monthly basis)
  • Responsible for obtaining individual's documentation from MSC's (Psychological, Medical, PPD's Psycho, LoC, DDP, ISP's, other).
  • COVID documentation, Telehealth coverage and assistance, and EVV implementation

Minimum Education: Bachelor's Degree preferred in a Human Services related field.

Minimum Experience: Must possess an un-restricted New York State Driver's License. At

least one-year prior experience working with the developmentally disabled. Leadership experience including supervising others, program management and experience documenting individual progress; Have some skills in community inclusion and excellent verbal and written communication skills. Must be fully vaccined.
