We are currently seeking an energetic, self-motivated Clinical Coordinator to join our growing Scatter Site team.

The Ideal Candidate:
- You have a Masters in Social Work or Masters in Psychology from an accredited college or university.
- You have three years minimum post master’s experience; 1-2 years of which should include supervisory experience
- You possess a working knowledge of mental health, substance abuse issues, and homelessness. In addition, you should be comfortable working with and around a diverse group of individuals.
- Must be proficient in Microsoft Word, Excel, and Outlook
Key Responsibilities:
- Major responsibilities of the Clinical Coordinator include, but are not limited to the following:
- Provides clinical and administrative supervision to members of the Clinical Team and interns.
- Offers assistance and support to Clinical Team via individual supervision, group supervision, and weekly team meetings.
- Receives NY/NY III Applications and sets up interviews for prospective residents. Facilitates admission meetings with the Clinical Team to discuss applicants and appropriate placement.
- Completes required governmental and agency reports.
- Provide individual counseling for residents and crisis intervention as needed.
- Assigns cases to members of the Clinical Team based on caseload and reviews service plans and progress notes to ensure completeness. Ensures that clients are seen regularly by Clinical staff, services are provided, client apartments are appropriately maintained, and other client care services as needed.
- Develops and maintains a system of assessment/evaluation of all case management functions, to assure compliance with The Doe Fund and Department of Health and Mental Health standards. Implements in-service staff training and maintains a directory of resources around issues such as substance abuse, mental health, and counseling.
- Collaborates with all departments as it relates to the residents and assists in coordinating care and referrals with community agencies and is involved in the discharge planning process from correctional or mental health facilities as necessary
- Other duties as assigned.
