Shift: First - Full-Time
Job Hours: 10:00 AM – 7:00 PM - Monday through Friday - Flexible hours.
Job Location: Norcross, GA (Gwinnett County)
View Point Health’s Mixtura Prevention Clubhouse located in Norcross, Georgia. Telehealth and in person services provided. Mixtura Prevention Clubhouse provides a motivational and safe space for youth between the ages of 12-17, and families within a diverse and culturally competent environment, to promote healthier life skills and habits to prevent the use of substances. Support youth to build on their talents and develop resilience through developing leadership skills, fostering relationships and increasing family community involvement.
We are looking for a bilingual Client Support Worker to join our multi-disciplinary team to provide client-support care and to assist with implementing our clubhouse programming based on prevention education program, recreational activities (sports, art, dancing, design), and outings.
Job Description:
- The Client Support Worker will interchangeably work with adolescents, caregivers, and family members.
- Provides family group skills interventions weekly to caregivers and family members of the client members of the clubhouse.
- Provides prevention education interventions to participants, via telehealth/in person.
- Documents client activities in ECR within 24 hours.
- Links caregivers with community resources according to their needs.
- Monitors client’s progress and compliance with the service plan as assessed by caregivers.
- Writes timely progress notes that relate directly to the curriculum activities per session.
- Identifies unmet needs, reviews available resources, and refer to the adequate services.
- Collects programmatic data (academic, assistance, behavior, surveys, etc.) and provides the information to Program Manager monthly.
- Assists with Open Access activities and client application process.
- Ability to cook nutritional meals to cohort members when in-person services deployed.
- Ability to drive a 15-passenger van and provide transportation when in-person services deployed.
- Ability to create fun and innovative programs, workshops, activities, field trips that support the progress of clubhouse members.
- Assists in the recruitment and development of Cohort members.
- Provides administrative support to Program Manager and any additional duties upon request.
Requirements/Competencies:
- Fully Bilingual and Proficient in English and Spanish languages.
- Bachelor of Arts or Science Degree in related area (Psychology, Sociology, or Early Childhood Intervention) Or High School Diploma.
- At least one year of experience working with children and adolescents.
- Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
- Candidates for selection must pass a criminal background check (including fingerprinting).
- Must pass a pre-employment drug screen and subject to random drug screens.
- Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Salary Compensations are based on Degree and Experience.
- Adheres to hours of continuing education annually; These can be achieved through internal (in-service) or external training; Provider is responsible for maintaining such hours for license/certification and for submitting documentation of them to Human Resources monthly..
- Other duties as assigned.
Qualifications:
- Bachelor's degree in a related area (Psychology, Sociology, or Early Childhood Intervention) from an accredited college or university OR High School Diploma.
Preferred Qualifications:
- Bachelor's degree in a related area (Psychology, Sociology, or Early Childhood Intervention) from an accredited college or university.
- At least one year of experience working with children and adolescent.
- Experience with Care Logic or other Electronic Medical Record (EMR).
- Is comfortable and proficient with collaborative & concurrent documentation.
- Experience in trauma-informed care.
