Job Description
This position is responsible for monitoring medication adherence rates and outreaching to patients, providers' offices, and pharmacies as needed to help support and coordinate improvement to medication adherence for our patients. This position requires the use of Excel, online payer portals, as well as different electronic medical record systems in order to gather the information needed to support the necessary outreach. This position may require some education to staff and providers on the medication measures and what fulfills or excludes a patient from quality measures. A wide degree of creativity and latitude is expected. At times, this position may be used to help support other quality initiatives as needed.
