SpringVale Health Centers (formerly Community Mental Healthcare) is seeking an experienced Children’s Case Manager to work with a designated caseload of children and adolescents with on-going mental illness to ensure quality of care, coordination of linkage to appropriate community services, skill development focus, monitoring of symptoms, and overall needs of the child are being met. The Children’s Case Manager will provide a variety of client-related functions to aid and support improved care coordination and functioning within the community while serving as an advocate for the client. Service areas includes preschool - Grade 12 within Tuscarawas County schools and local community. This is a full-time non-exempt hourly position working 40 hours per week.

Qualifications:

Bachelor’s degree in Social Work, Psychology, Nursing, or other mental health field plus 2 years post degree mental health or social work-type professional experience. Valid Ohio Driver’s License is required. Candidates must have strong communication skills, excellent time management, display decision-making capabilities, and have prior experience providing case manager to youth and adolescents. This position requires self-sufficiency, strong time management skills, and maintaining professional demeanor interacting with parents, teachers, school personnel and school administrators. Minimum of two (2) years related experience is preferred.
