Overview
The Care Coordinator works as part of a multi-disciplinary team coordinating services for children and their families/significant others. The Care Coordinator works with children and their support circle, including family members, other natural supports, care providers, and other systems to identify goals and assist in the planning, engagement, and coordination of resources needed to achieve those goals. This position helps engage the team of people who are providing services and supports for the child and family and ensures all services and supports are aligned towards meeting the treatment goals. The services provided utilize a Wraparound model of care that is family-driven and youth-guided.
Responsibilities
Responsibilities include Coordinates services and supports between individuals, their families, treatment agencies, and community partners. Works to eliminate barriers to care. Performs diagnostic assessments and formulates diagnoses. Develops service plans in conjunction with the client and other individuals, as indicated. Assesses symptoms and manages immediate crisis, including danger to self or others, and implements a crisis intervention plan if necessary. Implement service planning goals, utilizing program-specific evidence-based practices that are respectful and inclusive of the cultures and beliefs with which a youth/family identifies.
Organizes, schedules, and facilitates Child and Family Team meetings. Works with contracted providers of care to ensure appropriate services are being offered to clients and that all care needs are being met. Helps to develop and update Wraparound Plans of Care that are individualized, collaborative, culturally, and linguistically responsive, strengths, and outcome-based. Provides ongoing services and supports in a manner that demonstrates the ability to embrace, value, and celebrate the diverse cultures of children, youth, and families. Manages time and caseload effectively. Submits timely documentation of services provided using Electronic Health Records and collaborative documentation.
Qualifications
This position requires a Master's degree in a behavioral science, and related experience.
Must be a QMHP on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.

Must be registered as a licensure intern with the State of Oregon or be licensed in Oregon as an LMFT, LCSW, LPC or clinical Psychologist.
In order to avoid potential conflict of interest, applicants for this position cannot have received services from Options for Southern Oregon within the last 12 months.
Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system. Proficiency in Word and other MS Office applications preferred.
Must pass state-required background and DMV checks; possess a valid OR driver's license and a satisfactory driving record. Candidate must be able to work independently and flexibly, under general supervision.
