FULLTIME, Non-exempt: 40 hours
Coordinated Care Services Inc (CCSI) seeks talented Child & Family Specialists to contribute their expertise in providing essential business services to our provider organizations and customers. As a Child & Family Team Specialist, you would be to provide strength-based, wraparound coordination to youth and their families who are at risk of out-of-home placement, have emerging or unmet behavioral challenges, and demonstrate the need for involvement with multiple systems of care. The Child & Family Team Specialist utilized an integrative care model that ensures that every client receives individualized support services to meet their unique individual needs within the least restrictive, most normalized environment that is clinically appropriate.
CCSI is partnering with Monroe County Youth and Family Partnership (MCYFP) to hire Child & Family Specialists. This position is employed by CCSI and works on behalf of Monroe County Youth and Family Partnership (MCYFP).
In this role, you will be
- Coordinating a comprehensive planning process that utilizes a family’s strengths, as well as formal and informal supports, to develop a Plan of Care using strategies outside the parameters of standard service delivery to meet a youth’s and their family’s needs.
- Partnering effectively and collaboratively with Monroe County Departments of Child and Family Services and Office of Mental Health, as well as the education system, medical and clinical providers, community resources, and informal supports.
- Embracing, demonstrating, and modeling Monroe County System of Care values.
- Ensuring family and youth participation in all aspects of planning, service delivery, and evaluation.
- Ensuring smooth transitions for youth and their families between systems and supports, through consistent communication, planning, and following up among involved systems, agencies, providers, and the youth and family.
- Completing other duties as assigned.
- A minimum of a BA/BS in Social Work or a related field with 4 years of related experience is required. MA in social work with 2 years of experience in a related field is preferred.
- Must obtain and maintain certification to administer the Child and Adolescent Needs and Strengths (CANS-NY) tool.
Additional Requirements
- Satisfactory completion of all required background screenings.
- Full vaccination against the COVID-19 virus. Reasonable accommodations will be made for qualified religious or medical/disability needs.
- Valid Class D New York State Driver's License and access to a motor vehicle to meet the transportation needs of the position.
- Ability to work flexible hours to meet the needs of families.
