Carey Counseling Housing Department is searching for a Case Manager.

The Case Manager is a full-time, position responsible for assisting individuals experiencing homelessness with mental health and substance abuse disorders. This individual will be mission driven, professional, and passionate about being a part of the solution to assist homeless individuals and families with mental health, substance abuse and or co- occurring diagnosis in Lake County. The Case Manager should also:

  • Be a compassionate, passionate, and dedicated individual.
  • Be able to work well independently and within the framework of a team.
  • Be flexible and creative in response to the challenges faced with the tenants/clients in our housing.
  • Be able to handle challenging situations that can sometimes arise.

Responsibilities include but are not limited to:

  • Coordinate with the Director of Housing to provide case management and supportive services to our project participants.
  • Assist and coordinate client intake and assessment.
  • Make referrals for general assistance, benefits, medical care, mental health care, community agencies/eligible services, substance abuse counseling/programs, employment services, etc.
  • Maintain a visible presence and contact with project participants.
  • Maintain up-to-date participant files and meet regularly with the Director of Housing for monthly file reviews.
  • Maintain treatment plans and goals to help obtain participant success measures.  Ensure complete data is entered and accurate in HMIS database and that all funding/program-required documentation is completed accurately and on time.
  • Be knowledgeable regarding resources, issues, and best-practices affecting our target population.
  • Be knowledgeable of HUD-funded grant/program requirements.
  • Be trained and informed in the Housing First approach, Motivational Interviewing, De-escalation techniques, Harm Reduction, Trauma-Informed, Client-Centered, and Client-Choice models of care
  • Attend weekly housing meetings.
  • Participate in Outreach events.
  • Maintain and network with other agency participants.
  • Enters Case Notes-and Case management services provided daily into HMIS Data System within 24-48 hours of service.
  • Participate in bi-weekly CoC meetings.
  • Other duties as needed/assigned.

Requirements and Qualifications: Graduation from an accredited college or university with a minimum of a Bachelor’s degree in a health related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPN’s must have fifteen semester hours of behavioral health and one year of behavioral health care experience. Individuals with a Bachelor’s degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen college-level semester hours of course work in behavioral health and at least one year of work experience in the behavioral health setting.
