Case Manager - Ryan White
Job description
CW WIlliams is looking for a HIV-Ryan White Medical Case Manager to join our team. This position will provide a range of client-centered, confidential services that link clients with core care services.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Conducts assessments and reassessments on an annual basis
- Creates care plans that match the identified needs of the client
- Ensures all clients referred to services are eligible prior to the date of service
- Provides care services linkage and re-engagement support to clients
- Conducts field visits as needed to Ryan White who have disengaged from care
- Conducts outreach services as needed
- Refers clients to appropriate services highlighted in the care plan
- Screens for income supports for benefits/entitlements to advocate on behalf of client
- Completes emergency financial assistance applications for eligible clients
- Completes payment requests as needed for copays and other out of pocket costs
- Documents encounters using designated client database accurately and in a timely manner
- Develops and maintains professional relationships with social service, health and governmental agencies
- Participates in continuous quality improvement efforts
- Participates in regular supervision meetings, team meetings, in house training sessions and conferences as needed
- Assists with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others
- Protects organization's value and manage risk by keeping information confidential
- Performs other duties as assigned
EXPERIENCE AND EDUCATION
- Associates degree in any Human Services field with at least four (4) years of case management experience
- Bachelors degree in any non-human services field with at least two (2) years of case management experience, preferred
- Two (2) yrs experience - Ryan White Program
- Bi-lingual, preferred
KNOWLEDGE, SKILLS, AND ABILITIES
- The ability to maintain accurate work records and access these records as necessary.
- Knowledge of HIV infection and related chronic diseases
- he ability to assess client needs, create care plans, and follow-up in order to address barriers and ensure care is continuous and comprehensive
- The ability to use computer and web-based systems (e.g., PC-based tools, Microsoft applications, Web-based applications)
- Knowledge of services covered under the Ryan White program as well as the standards of care
- The ability and willingness to assess and establish priorities
- The ability to assess client needs, create care plans, and follow-up in order to address barriers and ensure care is continuous and comprehensive
- The ability to use computer and web-based systems (e.g., PC-based tools, Microsoft applications, Web-based applications)
- Knowledge of services covered under the Ryan White program as well as the standards of care
- The ability and willingness to assess and establish priorities
- The ability to communicate information clearly and politely to co-workers, supervisors, and guests when speaking
