Eyerly Ball Community Health Center (CMHC) was recently awarded a two-year federal grant to offer Rural Outreach Services to a five-county catchment area of Greene, Madison, Marshall, Jasper, and Poweshiek counties in Central Iowa. The Behavioral Health Navigators will have the opportunity to support the grant goals by working with the project team and grant clients to explore alternatives to hospitalization, higher levels of care, homelessness and incarceration; advocate for clients to help ensure their needs are met; collaborate with behavioral health and medical staff within the project area; and provide follow-up case management to clients as needed. Requirements:

1. Bachelor's degree in a Human Services field

2. 1 year of experience in mental health

3. Valid driver's license

4. Ability to work effectively as part of a team
