Crossnore School & Children's Home seeks a Case Manager II at its Avery Campus location in Avery, NC.

As a hub for service coordination, communications, and interventions for children in care, the Case Manager II is responsible for helping build coping skills in all areas of life, including emotional, medical, financial, and psycho-social. Case Manager II is responsible for screening and interviewing potential clients, developing action plans and goals with clients, connecting clients with resources, assisting with problem-solving as well as providing support and monitoring client success. Case Manager II may also supervise other case managers.
Status: Full Time

Hiring Range: $38,000 - $43,500 annually

JOB TITLE: Case Manager II - Residential

REPORTS TO: Residential Supervisor

SUMMARY: Provides individuals and families with service coordination and interventions that build coping skills in all areas of life including emotional, medical, financial and psychosocial. Responsible for screening and interviewing potential students, developing action plans and goals with students, connecting students with resources, assisting with problem solving as well as providing support and monitoring success. Ensures accurate and efficient provision of communication regarding the student to and from all members of the team.

STATUS: Full Time, Exempt
HIRING RANGE: $36,000 - $43,500 annually

QUALIFICATIONS
Education/Experience
  • Bachelor's Degree in Social Work or related Human Services field required with one year of directly related experience; or Bachelor’s Degree in another field with two years of directly related experience.
  • Previous work experience with residential programs preferred.
  • An equivalent combination of education and experience may be considered.
Skills/Competencies
  • General knowledge of the state child welfare and mental health systems required.
  • Considerable knowledge of family and group dynamics, a range of intervention techniques, resources in the community, and regulations and policies which impact youth.
  • Demonstrated passion for serving youth and families, and the youth-serving workforce.
  • Ability to establish and maintain effective working relationships with members of caseload and their families, coworkers, and community collaborators.
  • Demonstrated proficiency in the use of a variety of computer software programs. Proficiency with Microsoft and Google products required.
  • Demonstrated ability to exercise discretion and independent judgment.
  • Demonstrated ability to operate well in crisis situations and knowledge of cultural issues that can affect care.
  • Ability to work on diverse teams and must be flexible and willing to work with children of all ages, races, nationalities, sexual orientations, gender identities, disabilities, and backgrounds.
  • Good interpersonal, communication and organizational skills.
  • Ability to laugh, be creative and have willingness to make and learn from mistakes.
  • Displays enthusiasm and confidence in taking on tasks and challenges.
  • Valid Driver’s License, and must be willing to travel to various job related locations.

  • Ensure the provision of quality services through participation in Crossnore Communities for Children Performance Quality Improvement (PQI).
DUTIES AND RESPONSIBILITIES:
  • Coordinates all relevant team members to create viable Plans of Care that comply with a client’s permanency plan and schedule reviews as required to focus on current issues or developments (7 day, 30 day, 60 day, 90 day).
  • Develops and maintains positive relationships and contact with family members, DSS, school staff, coworkers, visiting resources, GAL’s and community resources to ensure communication and teamwork.
  • Ensures that all necessary and relevant documents are present in the client file and electronic health record (birth certificate, social security card, insurance information, CBA’s, POC’s, change over and monthly notes etc.) Seeks documentation of client's medical, dental, psychological, clinical, and educational history in order to gain a full perspective of history to more appropriately serve the client's current and future needs.
  • Ensures that the following are entered accurately and timely into the client’s electronic health record: weekly changeover notes, admissions and discharges, and a minimum of one comprehensive monthly note for each student on caseload.
  • Receives and responds to calls requesting placement.
  • Provides clear, accurate and timely communication to family, court and team members while adhering to confidentiality policy.
  • Coordinates financial services from DSS, family members or community resources to provide the client with adequate clothing, supplies, and financial support.
  • Works with team members and Supervisor to individualize program services where needed to ensure necessary living skills, spiritual development, emotional support, physical growth and structure meet each student’s needs.
  • Attends court as needed.
  • Provides transportation for clients to appointments, visits, court, etc.
  • Provides on campus behavioral support and collaborates with Clinical Services team to address behavioral challenges to help to prevent the need for a higher level of care and/or to support placement stability.
  • Provides On Call coverage with Residential staff for after hour intake referrals, hospital coverage, descalation situations, etc.
  • Provides supervision to clients during field trips and on-campus events.
  • Assists and/or leads client activities such as after school clubs and life skills classes.
  • Maintains contracts with local JCPC agencies; monitors and tracks available funds; attends JCPC meetings on a monthly basis.
  • Participates in agency meetings and monthly supervision.
  • Attends training to enhance professional growth (at least 24 hours annually).
  • Other duties as assigned.
DAYS/HOURS OF POSITION and WORK CONDITIONS:
Typical hours are 1st shift, Monday-Friday, though the schedule is flexible based on client needs. Must be willing to work some evenings and weekends as dictated by the demands of the position, including client appointments, field trips, hospital coverage, admissions, on-call coverage, and on-campus support. Work locations range from human services agencies, clinics, hospitals, or offices, and often involve visits to client’s homes. May involve contact with clients and/or family members who may be hostile, resistant, and violent.

Position may require lifting to 20 pounds at a time with frequent lifting or carrying of objects weighing up to 10 pounds. Frequent walking, standing, and sitting. Frequent reaching and/or grasping using hands and/or arms. May be required to ascend/descend stairs. Frequent use of hands requiring dexterity in using the telephone, computer keyboard, or other objects. Visual ability including distance, peripheral and depth perception. Ability to drive. Regularly required to talk and hear.
