The Case Manager I, Community Recovery provides case management services to clients of Catholic Charities. The Case Manager I, Community Recovery provides assessment and long-term support to families as they rebuild their homes and their lives after a natural or man-made disaster. The Case Manager I, Community Recovery determines appropriate financial assistance, including but not limited to, rental subsidy assistance to obtain long-term housing; provides credit counseling and financial education; guides participants in the development, implementation, and monitoring of individualized opportunity plans; provides assistance with finding and accessing appropriate community resources; and assists with identifying and overcoming barriers to services and achieving goals.

Essential Functions

  • Work in collaboration with other staff to triage and screen all clients to determine the need for recovery case management.
  • Maintain confidentiality regarding client information, sharing information only through the agency Release of Information (ROI) process.
  • Assist with outreach when needed to identify persons in need of services and referrals related to disaster recovery.
  • Complete interviews via phone or in person to provide prompt and responsive initial assessment of applicant needs; conduct a comprehensive, individualized, strengths-based, trauma-informed, and culturally responsive review of each client’s disaster recovery needs and related available resources.
  • Engage clients to cooperatively participate in the development, implementation, and ongoing review of an individualized disaster recovery plan related to needs surfaced in assessment.
  • Provide education and information to assist clients in effectively accessing the resources available in accordance with the sequence of assistance for disaster recovery.
  • Organize and prepare information and reports for meetings, briefings, and conversations with clients and staff.
  • Act as an advocate for and liaison between clients and other resource providers and organizations. Travel as needed to provide face-to-face services to clients.
  • Monitor client progress toward recovery goals; record relevant information using standardized forms and enter them into DataLink in a timely manner.
  • Work closely with all internal program staff to provide relevant services, as well as with partner agencies in the Sonoma County Long Term Recovery Group and potentially other counties within Catholic Charities’ service area.
  • Support the Mission, Vision, and Values of Catholic Charities in the performance of all essential job functions and responsibilities.

Note: Catholic Charities considers this position to be a mandated reporter of child abuse and elder abuse.

  • Work with Diocese of Santa Rosa and local parishes as appropriate and requested.
  • Perform other related duties as assigned.

Education, Experience, and Skills Required

  • High school diploma or High School Equivalency (HSE) required; a minimum of three years of successful experience in similar position or related field or demonstrated knowledge to perform job duties and enrollment in related classwork or related field such as community health, public health, social services, or psychology preferred.
  • Ability to work under pressure with a positive, solution-oriented approach; ability to work independently and possess strong time management skills as well as ability to work as part of a team.
  • Computer literacy required, including experience with Microsoft 365 Suite including Excel.
  • Previous experience understanding the causes of poverty and vulnerability, working with, and providing service to fragile and/or vulnerable clients from a variety of economic, social, diverse cultures, value systems, and cultural backgrounds in an appropriate, sensitive manner.
  • Strong written and verbal communication; organization, conflict resolution and problem-solving skills.
  • Excellent interpersonal skills, cooperative, friendly, proactive, and helpful attitude with clients and co-workers. Ability to work closely with clients and other employees to ensure a positive and constructive environment within the program or department and throughout the Agency.
  • Passion and enthusiasm for the mission of Catholic Charities and its clients.
  • Valid driver license and at least state required minimum of automobile insurance, clear DMV driving record and ability to provide own transportation. Must be willing and able to drive and travel throughout Sonoma, Napa, and Lake Counties.
  • Must pass DOJ background clearance (fingerprinting) in accordance with the Diocese of Santa Rosa’s policies prior to start of employment.
  • Bilingual (English/Spanish) preferred, but not required.
