Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include CrisisCARE, adult and child psychiatric, substance abuse, case management, residential, and therapy programs, as well as career development and wellness services. Zepf Center provides a full continuum of care which includes a trauma-informed environment for both patients and staff.


We are currently seeking Full-Time Case Managers within different departments at Zepf Center.
The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing mental illness and to ensure their needs are addressed. The Case Manager assists clients with the development of short and long-term goals and acquisition of community resources. Services will be provided with respect and in accordance with client’s rights.

The person in this position participates in development and coordination of the comprehensive care coordination and treatment plan, advocates for persons on their caseload, provides for assessment, linkage and referral, coordinates crisis intervention services and assists in the management of psychiatric and physical health symptoms. Levels of support required by people on caseload may vary greatly.


Essential Duties of this position include but are not limited to:

  • Assesses client strengths and needs. With the client and applicable team members, develops a comprehensive care coordination and treatment plan. Monitors and evaluates client progress toward goal attainment. Reviews and modifies plans every 90 days.
  • Maintains contact with the client as indicated in the treatment plan or as needed to ensure stability at least every 90 days or as required by regulatory bodies. Communicates with and provides support to clients in achieving goals and outcomes. Provides active outreach as necessary, including crisis intervention and management, and acquisition of appropriate community supports.
  • Links and/or refers clients to other services, programs, and agencies as needed. Assists in insuring clients obtain and retain applicable entitlements and follows up on same. Serves as an advocate for clients. With appropriate releases, communicates with family members and significant others as indicated.
  • Confers with supervisor and other team members regarding clients and, as appropriate, other agencies with emphasis on provision of most appropriate, effective and cost efficient services.
  • May provide face-to-face contact with clients who are hospitalized to facilitate discharge planning and coordination of services. Works with hospital and agency medical staff regarding same.
  • Assists clients in tracking and keeping appointments.
  • Completes required paper work in accordance with timelines and standards required by the agency, regulatory bodies and payors. Demonstrates excellent written and oral communication skills.
  • Carries out policies and procedures of the agency; may participate in community work, staff development, agency-wide projects; serve on agency committees/teams, attend meetings and performs other related duties as requested.

Work hours: Monday-Friday 8:30am -4:30pm.

Hybrid remote available after 4 months if engagement standards met

Requirements:
  • High School Diploma or GED required with demonstrated mental health experience;
  • Three-years of mental health experience in behavioral health - preferred.
  • CDCA certification required for SUD (substance use disorder) positions
  • Must have valid drivers license and be able to provide proof of auto insurance
  • Bachelor's degree welcome but not required

WORKING CONDITIONS

This job is a fast paced position that include a high volume of individual and family contact, collaboration with various community agencies, as well as computer entry and tracking and requires cultural/racial diversity and sensitivity. Employee must be able to maintain a patient engagement standard.

Employee's work is conducted in offices, participant's homes, and other community sites. Employee has contact with a wide range of people including some exhibiting anger and frustration under stressful conditions. Participants may be drug or alcohol-affected and in need of treatment. The employee will be exposed to and need to deal with a range of emotionally-charged issues in a compassionate, trauma-informed manner.

Must have a valid driver's license and acceptable driving record.
