Purpose Statement
The evaluation of work performance is an ongoing process. The purpose of the job description and written performance appraisal is to document a summary of the individual’s performance.

Job Summary
The Case Manager is responsible for working directly with the housing services clients to identify each individual/family’s strengths and specific needs. The case manager then assists each individual/family with connections to resources, skill development, personalized goal setting, and elimination of potential barriers to housing stability.

Job Responsibilities

• Strong working knowledge of computers, emphasis on Word, Excel and PowerPoint
• Ability to consistently maintain client confidentiality
• Develop an understanding and implement the Housing First Model
• Ability to work independently
• Maintain effective working relationships with and knowledge of community resources, both internal and external
• Responsible for accurately entering client data into HMIS system, training will be provided
• Develop relationships with area landlords and local Housing Authority in order to facilitate client placement
• Effectively communicate with the Homelessness Liaison for all area public schools
• Complete participants files within required deadlines
• Maintain detailed, up to date, and concise case notes on all interactions with clients and on behalf of the clients
• Participate in meetings and trainings as required
• Responsible for HUD service coordination caseload of clients, conducted at various locations in the community
• Identify, assess, plan, stabilize, and support clients in all housing programs
• Assist participants in the housing search process, included helping participants identify suitable housing options, and advocating for participants with landlords, and assisting clients with paperwork.
• Represent the Empowerment Center at events or on committees
• Travel into Clinton County and Jackson County to meet with clients. Locations including, but not limited to, the Clinton County Resource Center, the Clinton County Jail, emergency shelter locations, other public venues as identified by circumstance.

• Demonstrate a knowledge and understanding of homelessness, trauma informed care, human trafficking, and other forms housing instability.
• Ability to work within scheduled hours, must have reliable transportation, and demonstrate punctuality and teamwork.
• Live non-violent lifestyle.

Requirements

• BA in Social Work, Human Services or related fields and/or a working experience equivalent to education.
• Completion of required trainings within one month of hire
• Annual Blood borne Pathogens, CPR, ACES, Bridges Out of Poverty, EIC
• Maintain Drivers License
• Maintain Adequate Insurance
• Clear background check, completed annually.
