We are looking for Full Time RN/LPN Case Managers for both our Home Health and Hospice programs. Our home health program has a regulatory requirement of one year minimum experience for both positions. There is no minimum experience required for Hospice positions.

Responsibilities may include:

• Evaluating personal health and home environment
• Providing education and resources to help you control negative symptoms
• Coordinating care
• Communicating progress with caregivers and doctor
• Providing skilled nursing services such as dressing changes, medication management or IV care

The base salary is negotiable depending on experience and we offer a competitive benefits package to full-time employees.
