Description/Job Summary

The Case Manager is directly responsible for creating an environment that reflects

the mission of the RHD/Fernwood while complying with the standards of the Philadelphia's Office of Homeless Services (OHS). Staff will be trauma informed and will work non-traditional hours if needed in order to meet the need of the residents and/or staff support.

Responsibilities/Duties

The position functions within a team that regularly monitors participants' progress weekly.

Creates and sustains effective service plans with measurable goals

Data entry of all case notes, housing updates and other documentation as required

Demonstrate working knowledge of all pertinent policies, procedures, and regulations of HUD, OHS, RHD and Fernwood services

Complete required daily documentation and reports of all forms (paper and electronic)

Assist residents in navigating transportation resources available to them

Encourage participants to participate in Shelter Savings Plan

Assist participants with budgeting and money management

Assist with running community meetings

Other duties as assigned

Required Experience

Experience with mental health and/or substance abuse disorders, medically fragile, various ages of

adult males, a history of domestic violence, and DHS/child custody issues is preferred.

Preferably one year Case Management experience

Required Education

Bachelor's Degree in Social Work or a Social Work related field

Required Qualifications

Skilled in the components of Microsoft Office and learning/utilizing various databases

Excellent written and verbal communication skills

Knowledge of case management services, working with a homeless population or in a residential

setting

Ability to problem solve cases individually and/or within a team setting

Federal and State criminal background check satisfying Philadelphia's Office of Homeless services

requirements.

Program Summary

Fernwood West Fernwood is a program with RHD's Philadelphia Behavioral Health and Housing that provides emergency housing services for homeless men year round according to our funder, Philadelphia's Office of Homeless Services (OHS). The Case Manager will work with one of the following emergency housing Units managed by Resources for Human Development (RHD). During the stay of the homeless men focus on the five domains of life: housing, employment, health, community involvement, and education. The primary goal of the program is for participants to obtain permanent housing in a short period of time.


Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within
8 weeks of hire (start date). This currently does not include boosters.
