Community Mental Health Center, Inc. is a provider of comprehensive mental health services, offering inpatient, outpatient, home-based, school, and community-based programs to individuals and families in Dearborn, Franklin, Ohio, Ripley and Switzerland counties in Southeastern Indiana. CMHC offers a full continuum of care to individuals and families of all ages. CMHC follows the recovery model which emphasizes and supports each individual's potential for recovery. Recovery is seen as a personal journey that utilizes supportive relationships, empowerment, social inclusion, and coping skills.

Position Responsibilities: The Case Manager will provide intensive family and individual case management services to children and their families involved in the child and adolescent Intensive Family Services Program. Provides therapeutic services for clients and families involved in Intensive Family Services including home based services, group counseling and Therapeutic Day Program services.


Qualifications:
Bachelor degree in Social Work, Psychology, or other mental health related field and one (1) year experience providing direct services to clients in a mental health setting. Relevant professional and life experience may also be considered in lieu of degree. Basic knowledge of needs of seriously emotionally handicapped youth and multi-problem families. Ability to work effectively with groups of children, adolescents, and families.


Day-to-Day Expectations:

  • Models appropriate problem-solving and conflict-resolution techniques.
  • Models cooperative relationships with other staff who provide Intensive Family Services.
  • Monitors family progress in treatment and assists in client/family evaluation
  • Coordinates communication and services provided by other CMHC programs, community agencies and schools.
  • Assists client/family in obtaining necessary resources and entitlements assistance.
  • Provides transportation to treatment activities and appointments, when necessary.
  • Teaches and models appropriate skills to client and family to encourage health family communication and roles.
  • Documents all clinical activity daily within the Electronic Health Record.
