Job Summary:
The primary mission of the Case Coordinator is to oversee and ensure the quality and comprehensiveness of record keeping for the individuals we serve to ensure all regulatory requirements are met and that services are of the highest quality. The Coordinator provides a myriad of tasks to support the overall success of individuals we serve. Maintain regular contact (via phone, email, mailings, meetings) with guardians, parents, residential providers and clinical staff. Attend and participate in all relevant coordination meetings both externally and internally, communicating overall goals of the department and individuals supported.
- Knows and understand profiles and histories of individuals served to ensure that appropriate goals are developed and implemented for individuals
- Meets with the individual and provider prior to the ISA meeting to ascertain their goals and wishes for the coming year
- Summarizes the previous year's goals and accomplishments
- Articulates all of the above at the ISA meetings
- Shift 9:00am-5:00pm or 8:30am-4:30pm Monday-Friday - Stratham NH Area
- High School Diploma required and 4 years experience in the Developmental Disabilities system
Oversees the quality of day and staffed program services
- Reviews daily notes for accuracy, quality and verification of hours
- Processes attendance forms, verifies them and uploads as needed
- Checks Monthly Progress Notes for progress toward identified objectives; signs off and uploads to designated area
- Monitors the spending/purposes of PEX and petty cash funds, and sees that appropriate documentation is stored
- Completes and files all documentation required by regulatory agencies, focusing on providing complete, relevant, timely, and accurate information.
- Maintains confidential records and ensures approval on plans and notes from appropriate parties.
- Participates in RFP process as needed.
- Assists with the Certification Process.
Oversees quality of clinical/behavioral services; responds to significant incidents
- Processes Incident Reports when appropriate, in a timely manner, and distributes copies as needed.
- Reviews behavior data as needed for comprehensiveness.
- Participates in: Aspire's CRC (Case Review Committee).
- Complete all mandatory trainings to ensure skill maintenance and development.
- Recommends trainings that may benefit the department or group.
- Participates in the training of peers on case coordination related duties as assigned.

A successful candidate will have a personal vehicle, proof of 100,000/300,000 liability auto insurance, and a safe driving record. Candidates must have a clean BEAS and a clean criminal record in order to attain employment with Aspire. A background check will be required for successful candidates seeking employment.
