Job description
PURPOSE/FUNCTION OF PROGRAM:
The REGIONS residential program will provide a home-like environment with a therapeutic based milieu for JJ involved male adolescents with a rehabilitation and treatment focus. The program is designed to reduce risk and recidivism and will take the lead on assisting the adolescent, family and stakeholders with the youth’s transition plan back into the community.
POSITION OVERVIEW:
This position establishes and maintains local educational connections and assists families with empowerment, especially around educational advocacy. They obtain educational records, identify needs, and assist the clinician in facilitating team meetings and help to establish long term meaningful and supportive connections with the community.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Coordinates provider meetings, treatment plan review meetings, and all other team meetings, and attends when possible.
- Coordinates educational services for clients, obtaining educational and health records, arranging transportation, and attending PPT and other school based meetings as necessary.
- Identify educational gaps/services.
- Assist the adolescent and family with school reintegration
- Conduct and/or support vocational planning
- Explore secondary options for/with the adolescent.
- Assist the program manager when necessary as well as maintain regular and open communication with the Probation Officer as to the adolescent’s progress, challenges and successes in regards to educational matters.
- Documents all services provided.
- Arranges flexible work schedule, including evening and weekends, in order to provide needed services to clients
- Collaborates with collateral contacts in order to get past records including any school documentation, documentation from past providers, and necessary CSSD paperwork.
- Assist family and probation officer with arranging after-care services.
- Works closely with other program staff to facilitate implementation of care plan.
- Assists in the gathering of QI measures.
- Attends staff meetings as required.
- Completes all required internal and external trainings including Relias by due date.
- Provides transportation as needed to youth.
- Accepts additional assignments and responsibility as requested by supervisor.
ESSENTIAL QUALIFICATIONS/SKILLS:
- Bachelor of Science in Education or Social Work or related field
- Must have experience working with educational systems
- Knowledge of diverse populations and family systems
- Must have effective communication skills
- Ability to proactively problem solve and develop solutions to barriers
- Valid driver’s license and clean driving record
- Fulfills competency-based requirements in Boys & Girls Social Skills Model and TCI, Risking Connections, and other approaches as identified

- Bi-lingual
SUPERVISORY RESPONSIBILITIES:
- N/A
PHYSICAL DEMANDS:
- Ability to perform TCI consistent with program policy and procedure
- Ability to climb stairs
WORKING CONDITIONS AND ENVIRONMENT:
- Travel is required to client’s homes and meetings
- Willing to maintain a flexible schedule to meeting client/program needs
