Care Manager

Responsibilities:

• Responsible for profit & loss of the Bay Area operation
• Being the primary care manager for assigned cases to oversee delivery of care services to meet clients' needs. Manages client’s health status across the continuum of care. Coordinates with physicians, staff and providers as needed to ensure client’s care
• Responsible for intake calls, follow up and update leads status;
• Conduct in-home visits, initial assessments and re-assessments of prospects and clients; develop or adjust customized care plans accordingly;
• Complete all necessary assessment and re-assessment documentation and maintenance of such in system;
• Interview and select caregiver candidates;
• Train all on-boarding caregivers;
• Continue to provide special and hands-on training to all caregivers on a regular basis;
• Work with CEO and other colleagues to recruit caregivers from various sources;
• Work with CEO and other colleagues to formulate company policy, procedure and practices in providing in-home care services.

Experience Required:

• Minimum Bachelor’s Degree OR experience and education as a substitute for minimum degree.
• Demonstrates knowledge of in-home care management, plan of care, measurable outcomes.
• Other skills include: analysis, assessment communication, counseling, interpretation of policy, interviewing, knowledge of applicable laws, networking, organization, planning, problem identification, resolution, research, scheduling, staff development, and reassessment of client clinical functioning.
