Reports To: Executive Director Salary Range: $52,500 - $57,000

ESSENTIAL FUNCTIONS
The Care Management Social Worker is responsible for the coordination, management, and reporting aspects of the Care Management program that provides support, information, and referrals for community-based services that support our participants in remaining in their homes.

Responsibilities

  • Daily provision of Care Management Programs and Services, including, but not limited to Emergency Funds Assistance (EFAs) and the Senior Intensive Supports program
  • Support the long-term development of the Care Management Program
  • Ensure that all deadlines and performance standards are met
  • Cross-train or train current and new staff on existing and new department procedures
  • Develop and strengthen partnerships within the community that will enhance the supportive services that
  • Supervise undergraduate and graduate school interns, when applicable

Miscellaneous

  • Passionate about working with and on behalf of older and/or disabled adults
  • Committed to supporting community members facing food and/or financial insecurity
  • Ability to schedule and manage tasks effectively
  • Expertise with Microsoft Office Suite
  • Must have a car available, a current driver’s license and insurance coverage
  • Evening/weekend hours as necessary
  • Other duties as assigned by the Executive Director

WORKING CONDITIONS
Hours: Full-time, 37.5 hours per week
Location: 9 S. Bryn Mawr Avenue, Bryn Mawr, PA 19010
Other: Support administrative needs, as required

QUALIFICATIONS

  • Master of Social Work required with at least 2 years of work experience (LSW desired)
  • Prior supervisory experience preferred
  • Conflict resolution and problem-solving skills
  • Excellent written and verbal communication
  • Experience working with older adults preferred
  • Experience assisting with filing for benefits such as LIHEAP, SNAP/WIC
  • Valid Driver’s License and good driving record
  • Personal vehicle or access to independent transportation
  • Proof of motor vehicle insurance
