Job description
Under the direction of the Manager of Care Management, this position assists the Care Manager, Registered Nurse and Social Worker with various tasks including low level and moderate level risk assessed patients with needs, such as setting up home health, durable medical equipment, nursing facilities. Conducts compliance and regulatory audits/interviews which include Medicare, The Joint Commission, DOH and other regulatory bodies as needed for IMM's; communicates the audit findings, conclusions and recommendation to the Care Management Administrative Director. This position also completes the second IMM at minimum two days prior to discharge and discharge notification for all third party payors, which includes appropriate documentation in Meditech. Identifies variances affecting utilization of hospital resources and quality of care, communicates to the appropriate staff and documents variances. This position also provides support to department in area of reports completion, schedules, payroll, minutes, patient portal and ordering supplies.
Qualifications:
High school diploma with extensive healthcare experience required; Bachelors preferred.
Experience:
A minimum of two years' experience in an acute health care setting preferred; computer, telephone and analytical skills required; organizational and time management skills are essential.
Mental/Physical Requirements:

May exert up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.

  • Sits on a hard or cushioned chair frequently.
  • Lifts up to 20 pounds to and from all levels (floor, waist, shoulder, overhead) on an occasional basis.
  • Carries up to 15 pounds occasionally throughout the hospital.
  • Bends, twists, squats, and kneels occasionally.
  • Reaches up to 2 feet overhead/in front of oneself on an occasional basis.
  • Near-visual acuity for deciphering fine print.
  • Manual dexterity adequate for utilizing a keyboard and calculator, peeling off labels and processing paperwork.
  • Ability to adapt to simultaneous, multiple and varied stimuli.
  • Auditory acuity for hearing telephone conversation, normal voice tones when not facing the individual, call bells, overhead pagers, etc.

Travels to other summit organizations to perform audit.

Mental Demands - possesses good general mental health, ability to work under pressure and function independently; ability to establish priorities among essential functions of the job and coordinate these functions with others; sound conflict resolution skills; ability to communicate in both verbal and written form; ability to speak, read and write the English language; ability to maintain good interpersonal relationships with staff, other hospital personnel, physicians and the general public. Must be self-directed. Position demands good organization and time management skills, flexibility, and the ability to efficiently and effectively handle multiple demands simultaneously, while working under the pressure of deadlines and shifting priorities.
