We work in partnership with human services, child welfare systems, juvenile courts, and community-based service providers so that together we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.
We are currently looking for full-time Care Coordinator:
Summary: The care coordinator will bring services and resources to clients to help achieve established goals by collaborating with the appropriate providers and agencies. This includes developing service goals and linkages with schools, and relevant community based organizations that enhance and ensure clients/ families are in receipt of optimal care and service coordination for addiction intervention, AODA counseling and recovery support services. The care coordinator may also be required to work with high risk caseloads, co-occurring clients and families.
Essential Duties and Responsibilities
- Provide specialized care coordination services to clients and their families with behavioral health concerns and other needs which may include addiction, mental health, etc.
- Works with high risk caseloads, co-occurring clients, families and children, providing assessments, client advocacy, prevention, and education services.
- Maintains a therapeutic alliance with clients through advocacy and resource assistance, including but not limited to medical health needs, housing, vocational services, benefits and entitlements.
- Screen and assess client for their appropriateness for agency services.
- Care Coordinator staff will be dedicated to cover specific geographic areas and possibly co-located at a designated provider site for integration at the community level to ensure that the needs of the youth and family are met;
- Care Coordinators will make follow-up calls to youth and family with weekly outreach which may include texts/emails and home visits;
- Participates in multi-disciplinary team meetings with TASC unit, treatment providers, probation, Lurie Children Hospital and attend regular supervisory meetings.
- Collaborates with the client to develop an individualized care plan and provide care coordination services.
- Coordinate outreach services to churches, schools, and community based social service agencies.
- Participate in community events to represent the agency and program.
- Plan, coordinate and facilitate community events for participants.
- Prepare, interpret and present reports regarding program operations, evaluation of service and utilization of resources.
- Represent the client and agency at school staffing, provider networks and court hearings.
- Provide referrals for identified needs of the client and crisis intervention when needed.
- Complete and maintain all required documentation which includes, documenting the results of assessments, service plans, reports, progress notes, discharge summaries and other client-related activities including accurate reporting of services.
Qualifications
- Master’s degree preferred in human services, social work, psychology or other related fields or two to four years’ experience.
- Licensed professional counselor possessing and licensed under the Professional Counselor and Clinical Professional Counselor Licensing Act [225 ILCS 107] with specialized training in mental health services or with at least two years’ experience in mental health services (preferred)
- Knowledge of human behavior for the assessment and signs and symptoms of substance abuse disorders.
- Knowledge of the signs and symptoms of substance abuse including pharmacological factors.
- Knowledge of the ASAM placement criteria and the DSM-Vas it relates to substance-related disorders or able to acquire the knowledge.
- Ability to examine and address client issues which includes substance abuse, bullying, depression, LGBTQ and/or suicide.
- Possess clinical skills to effectively manage clients.
- Ability to obtain CADC certification through the State Certification Board (IAODAPCA) within the first two years in the field (if required by program). MISA certification is optional but encouraged.
- Knowledge of the Code of Ethics for mental health and substance abuse professionals.
- Knowledge of the federal drug and alcohol confidentiality law (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act.
- Knowledge of Rule 132 documentation, Medicaid billing protocols and DASA regulations;
- Ability to work flexible hours and travel as needed (evening/weekend hours)
- Ability to work with a diverse population.
- Knowledge of computers, including proficiency in Microsoft Office applications and the Internet.
- Must be available to attend meetings and meet the needs of the program and/or clients throughout the city, counties or wherever needed (a reliable car, insurance and valid driver’s license may be required for the job).
