Care Coordinator Supervisor

Unison Health is seeking a full-time Care Coordinator Supervisor to lead a team of individuals in the Ohio Rise Program. This position is responsible for promoting the recovery of children/families participating in the Ohio Rise program through the supervision of direct service care coordinators. Our service area includes Lucas, Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert and Mercer counties.

Education/Experience/Other Requirements:

• Bachelor’s degree in social work, psychology or related field with a master’s degree preferred.
• LSW, LPC, LISW or LPCC preferred.
• Minimum three years’ experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
• Must be proficient in the use of computers and software.
• Possess a valid driver’s license, reliable transportation, be able to be insured through the agency commercial policy and have personal auto insurance.
• Prior supervisory experience preferred.

Duties:

• Implement the required model of care, provide individualized care based on client need, and link clients to needed services and supports.
• Ensure assigned staff participate in ongoing training, and coaching to ensure consistency in delivering care coordination.
• Monitors completion of paperwork by assigned staff, including initial assessments, CANS assessments, care plans, and discharges.
