POSITION SUMMARY
Under the direction of the Director of Care Coordination and Patient Transitions, the Care Coordinator II is responsible for coordination and management of the hospital based population. As a leader of the multidisciplinary care management team, the Care Coordinator II is accountable and responsible for coordinating care and service of a selected patient population, promoting effective utilization and monitoring of health care resources, providing guidance to the interdisciplinary team to achieve desired clinical, financial and resource outcomes. Provides employee assistance, consultation to staff, physicians, and community providers regarding psychosocial aspects of care.
BASIC KNOWLEDGE
  • Demonstrates ability to contribute to interdisciplinary psycosocial assessment and recommendations for plan of care
  • Individual and family supportive counseling and advocacy.
  • DRG’s level of care, regulations related to continuing care and
  • Interdisciplinary collaboration requested for staff and community education.
  • Proficient use of verbal and written skills essential.
  • Cross trained to manage all hospital departments.
  • Shows ability to participate in Community Network system. Contributes to overall trend reporting and data collection for quality management reports.
MINIMUM REQUIREMENTS
Preferred MSW or BSN RN with acute healthcare setting experience, MA in related field acceptable with acute healthcare experience.
Will consider experienced LPN/RN with acute and/or Skilled Nursing Facility healthcare setting experience, or Bachelor’s level working towards a Master’s level degree in Social Work or Counseling.
EXPERIENCE
3-5 years of clinical experience. Fulfills annual hospital mandatory requirements for safety, MSDS, infection control. Demonstrates leadership qualities.
