UnityPoint Health – Eyerly Ball is seeking Care Coordinators in our Des Moines, Iowa location to join our team. Full-time opportunity, Monday-Friday schedule.

As a Care Coordinator, you will partner with adults with serious mental health and assist with client behavioral health, physical health and social factors through a variety of modalities. In this fast paced environment, the Care Coordinator assesses the client’s needs to develop and implement an individualized service plan designed to improve the client’s overall health and quality of life.

As a Care Coordinator Behavioral Health Professional you will:

  • Provide outreach activities to support and engage clients in care coordination.
  • Conduct individualized, comprehensive whole person assessments.
  • Coordinate treatment with a multi-disciplinary team and the client’s support team, including primary care providers and specialists.
  • Promote client’s health and ensure that all personal health goals are included in person-centered care management plans. Provide resources, referrals or coordination to social and community services available.
  • Provide self-management support and development of self-management plans and/or relapse prevention plans so that clients can attain personal health goals.
  • Promote self-direction and skill development in the area of independent administering of medication and medication adherence.
  • Advocate for the client’s family members, including recommendation of support services.
  • Work with managed care organizations to verify continued client eligibility and funding.
Requirements:

Ideal Candidate:

  • Capacity to professionally relate to a wide variety of clients, as well as a desire to serve as a client advocate.
  • Ability to practice good verbal and written communication skills with ability to complete case notes timely and other required documentation.
  • Capacity to engage with clients effectively, patiently and respond professionally at all times.
  • Ongoing passion about making a difference in the lives of those who have been touched by mental illness.

To be considered you’ll need:

  • Bachelor’s degree in social work, psychology, human services or related field.
  • One year experience working with population served.
  • Possess a valid driver’s license.
  • Required to maintain a minimum of $100,000 and $300,000 per accident auto insurance liability coverage. Required to provide proof of coverage.
