Job description
The Care Coordination Assistant supports the discharge planning process by communicating with and coordinating post discharge services and providers as requested by the Registered Nurse/Social Worker Care coordination team.
These post discharge providers will include but are not limited to: Home Health, Hospice, ALF/RH/SNF/ARF placement, DME, transportation, doctor office visits, etc.
Additionally, the Care Coordination Assistant supports the Utilization Review function of Care Coordination by serving as a liaison between the RN Care Coordinator and external payers, monitoring work queues, submitting of timely information, etc. Licensure, Certification, or Registration Requirements for Hire: N/A Licensure, Certification, or Registration Requirements for continued employment: N/A Experience REQUIRED: Minimum of two (2) years of healthcare related experience Knowledge of computers and various computer programs such as Microsoft Suite products, [especially, Word and Excel] and Adobe Professional Experience PREFERRED: Three to five (3-5) years experience in acute care academic center or comparable organization Education/training REQUIRED: High School Diploma or equivalent Education/training PREFERRED: Some college level education Independent action(s) required: Coordinates the communication and referrals to post discharge providers including home health, home and inpatient hospice, Long Term Acute Care facilities, Skilled Nursing facilities, Assisted Living facilities, DME, transportation and any other type of post discharge provider of services required. Position: VCU Health at Home Care Coordination Assistant – Coordinated Care & Continuum Integration
Shift: Rotating Day/Evening/Night/Weekends and Holidays
VCU Health is seeking authentic, passionate and inspiring candidates to staff a new, virtually-enabled Hospital-at Home program, housed in the health system’s Continuum Integration Center in north Richmond. This exciting opportunity offers innovation and professional growth and you would be joining an already incredible team, which is expanding to provide this new service offering under the highly-regarded VCU Health at Home brand.
Located in contemporary offices on Richmond’s Northside, this on-site position will support the continued growth and development of a robust VCU Health at Home strategy, including 24/7 acute virtual monitoring of individuals enrolled in the VCUHS Hospital at Home program, transitional remote patient monitoring, and triage support for both high-risk ambulatory and post-discharge patients across VCU Health System Hospitals.
Hospital-at-Home programs are gaining momentum nationwide, because of the benefits for patients and health systems, including increased patient satisfaction, improved patient outcomes, better performance on value-based metrics and relieving facility capacity constraints. The model operates with a patient-centric focus and a comprehensive, team-based, outcomes-motivated approach to care using population health principles to keep patients healthy.
The focus of the position is:
- To assist the VCU Health at Home clinical team in ensuring quality coordination of care for both acute (Hospital at Home) and post-acute (transitional remote patient monitoring and triage support line) patients throughout the VCU Health System.
- To ensure quality patient care across the continuum, reduce avoidable readmissions, and improve outcomes for both acute inpatients and the highest risk discharged individuals from VCU Health System.
- To ensure safe quality enrollment in to both the Hospital at Home program and the transitional remote patient monitoring program by completing accurate data entry in to both an electronic medical record and a medical platform.
- To provide quality virtual support to the VCU Health at Home clinical team through screening of calls, telephone outreaches, participating in interdisciplinary rounds, setting up additional home services and appointments, and escalating both clinical and non-clinical needs appropriately.
- To ensure medical and nursing care is set up for individuals enrolled in the Hospital at Home program by managing multiple schedules for both virtual and in-person home visits.
- To ensure inpatient individuals are receiving appropriate services during their Hospital at Home stay including but not limited to therapy, food, consultative, mobile imaging, labs, transportation, and medical equipment.
- To provide quality transitions of care when a higher level of care is needed by assisting nursing and/or care coordination with the transportation and escalation process.
- To successfully collaborate with the interdisciplinary and care coordination team in discharge planning and setting up appropriate resources to prevent un-necessary hospital readmissions, such as home health, transitional remote patient monitoring, or follow-up PCP/Specialist appointments.
- To ensure quality customer service when screening telephonic and virtual communication.
- To ensure quality interdisciplinary communication between internal and external health care teams by resourcing the electronic health medical record, scanning of documents, connecting care teams virtually, and assisting with keeping the medical record up to date.
