The Breast & Cervical Cancer Program Navigator’s responsibilities include providing outreach services, assisting patients in navigating healthcare systems, reconciling electronic medical records, and following up on abnormal screening results. The primary goal of the position is to increase the number of uninsured or underinsured patients screened for breast and cervical cancer and to promote breast and cervical cancer prevention and early detection awareness in the community.

ESSENTIAL JOB DUTIES:

· Schedule appointments and provide reminders for patients due for cancer screening

· Work with multi-disciplinary team to provide culturally appropriate patient navigation to AI/AN people and other vulnerable populations

· Collaborate and work in partnership with medical provider care teams to improve patient care, address health disparities and promote health equity

· Assist patients in addressing social determinants of health while navigating patients through breast and cervical cancer screening. Schedule transportation and work to overcome other barriers.

· Participate in multi-disciplinary staff meetings

· Timely data entry and documentation of patient navigation in Chronic Disease database and Electronic Health Record.

  • Serve as a subject matter expert in breast and cervical cancer prevention and early detection
  • Provide outreach at Saturday Clinics, evening clinics, powwows, elders’ dinners, and other cultural events to identify and recruit eligible patients.
  • Assist with preparation for specialty clinics and events.
  • Maintain and develop partnerships with community stakeholders.
  • Demonstrate a patient focus, respond to the needs of those being served; represent NARA with courteous attention, respect, and dignity.
  • Work closely with Program Coordinator to contribute to program efficiency and effectiveness.

Attend trainings, conferences, workshops and other meetings as directed.

Maintain communication with patients, survivors, families, and provider care teams to monitor patient satisfaction.

Pick-up and transport program supplies as needed.

Schedule and provide occasional transportation for patients to medical appointments

Arrange interpretative services as needed.

Perform other duties as assigned

QUALIFICATIONS:

  • High school diploma or equivalent
  • Two years’ experience in a health-related field and community resource navigation for the underserved and diverse populations
  • Experience in cancer patient navigation preferred.
  • Evidence of computer proficiency in Microsoft Office Suite (Word, Excel, Access, Outlook, and PowerPoint)
  • Experience with Electronic Health Records and detailed data entry.
  • Experience with Centricity/Athena EHR and database management (Med-IT) is a plus.
  • Availability to work evenings, weekends, and special events.
  • Availability to travel to CDC conferences/trainings as needed.
  • Experience working with diverse populations and familiarity with motivational interviewing and trauma informed care principles.
  • Ability to walk, sit and stand for prolonged periods of time.
  • Ability to use hands to finger, handle, or feel; and reach forward with hands and arms.
  • Ability to twist, bend stoop, kneel, or crouch.
  • Ability to lift and/or move up to 25 pounds.
  • Evidence of ability to convey information clearly and effectively
  • Ability to maintain professional boundaries with clients, the community, and other staff members.
  • Experience working within the Native American/Alaska Native community preferred.
  • If in recovery, must have a minimum of two years sobriety/clean time.
  • Ability to travel between multiple sites and location in a day.
  • Ability to pass a pre-employment or for cause drug tests.
  • Ability to pass criminal background and DMV checks.
