JOB SUMMARY:

The Outreach Coordinator (OC) is responsible for assisting with Foremost Family Health Centers community outreach events and efforts. Performs community outreach and marketing.
Increases the number of residents who have access to primary and preventative health care services by identifying those eligible for publicly sponsored insurance programs and other social services and facilitating the enrollment process. Indirectly reports to the Lead Outreach
Coordinator and directly reports to the Chief Organizational Development Officer.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

Community Relations and Advocacy

  • Attends community events and provide feedback to community partners on community events
  • Attends partner meetings and actively participates in partner meetings
  • Represents FFHC in local meetings as requested
  • Participates in National Association of Community Health Centers (NACHC) and Texas

Association of Community Health Centers (TACHC) activities and disseminate relevant information to team members

  • Establishes relationships with new community organizations (i.e., business, housing, government, education, church, public health, etc.)

Events Management

  • Assists with the planning, implementation, and evaluation of FFHC events
  • Solicits feedback on FFHC events and identify opportunities for improvement
  • Communicates and collaborates with FFHC departments regarding logistics including volunteers and event staffing

Marketing and Communications

  • Updates community outreach calendar, as needed
  • Creates and posts social media content promoting FFHC events and initiatives
  • Promotes FFHC events through email blasts, social media, word-of-mouth, etc.

Patient Outreach & Enrollment

  • Maintains PPACA enrollment certification annually
  • Ensures newly enrolled members of publicly funded insurance programs understand how their insurance plan works and have a functional relationship with a primary care provider

(PCP)

  • Assists patients with enrolling in CHIP or Medicaid, as needed
  • Assesses patients’ Social Determinants of Health (SDOH)
  • Connects patients to various FFHC and external resources (i.e., hypertension program, asthma program, Aunt Bertha)
  • Schedules patient appointments and/or provides an overview of the sliding fee discount program, as needed

Other

  • Other duties assigned by management within the skill level of the employee

Experience:

  • Prior work in Health Care Industry (preferred)
  • Work as a community health outreach worker or enrollment specialist highly desirable

Knowledge:

  • Bachelor’s degree in community health, health education, or another related field

(preferred)

  • Working knowledge of health and social services (preferred)
  • Working knowledge of social media platforms (e.g., Facebook, Instagram, Twitter, etc.)

Minimum Qualifications:

  • High school graduate or equivalent
  • Minimum two years working in a primary care facility
  • Obtain and/or maintain CHW certification from TDSHS Community Health Worker

Program

  • Abilities: Must be able to speak, read, write, and comprehend the English language. Must be able to stand/walk for majority of the shift. Must be able to bend or stoop. Must be able to lift 10 lbs. unassisted. Must be able to physically assist patients that may be in physical distress, fainting/falling or provide general assistance if mobility of patient is impaired. Must have good vision (e.g., near acuity and depth perception)

OUTREACH COORDINATOR

  • Other: o Must be at least 21 years of age o Must be able to work with diverse groups (e.g., children, adults, different cultures, etc.) o Must have current Texas Driver’s License and a clean driving record o Must have reliable transportation o Must have proof of automobile liability insurance o Must be able to periodically work on Saturdays or Sundays o May require some local travel between different clinic locations o Must be able to maintain required immunizations as required by the organization
