JOB TITLE: Behavioral Health Consultant/Primary Care
SUMMARY: The Behavioral Health Consultant works as a member of the Interdisciplinary Treatment Team, providing a full range of evidence-based mental health services to patients with behavioral health problems within the primary care setting. As a cooperative team member, the Behavioral Health Consultant assumes responsibility for all aspects of the mental health care of these patients. The Behavioral Health Consultant provides evidence-based treatment consistent with their scope of practice and licensure status. An essential function of the position will be the development and monitoring specific behavior change plans documented in the patient’s Individual Comprehensive Treatment Plan. In addition, the Behavioral Health Consultant will provide mental health education and training to the other members of the Interdisciplinary Treatment.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following, other duties may be assigned:
Assessment & Treatment Planning – 75%
- Serve as a member of the Interdisciplinary Treatment Team, working with primary care providers in diagnosing problems, formulating treatment plans, and evaluating progress.
- Assess clinical status and offer treatment recommendations of patients referred by their primary health care provider.
- Provide evidence-based individual and/or group therapy for patients and their families.
Documentation & Reporting – 15%
- Prepare socio-psychiatric case history information, psychosocial assessment and treatment plans.
- Make referrals to mental health specialists as appropriate.
- Participate in the detection of “at risk “patients and develop treatment plans to prevent further psychosocial, physical deterioration, and/or relapse.
- Maintain records and prepare verbal and written reports and correspondence related to patient care.
In-service Training & Community Outreach – 6%
- Participate in workshops, meetings, and conferences.
- Participate in mobilizing community resources and maintain cooperative relationships with community agencies, schools, and courts.
- Provide in-service training.
- Serve on committees.
Other Duties - 4%
- Participate in the evaluation of peers and support staff.
- Monitor the behavioral health program and make suggestions to improve patient care and outcomes.
Supervisory Responsibilities: This job has no supervisory responsibilities.
Education: Master’s degree in Social Work.
Certificates, Licenses, Registrations: State of Michigan - Licensed Master’s Social Worker,
CPR certification and State of Michigan Driver’s License.
