BH Support Services: Starting Pay Range: $18.70 - $20.90 an hour

Job Summary:


  • Provides supportive services within the Behavioral Health team to facilitate access to behavioral health services for the patient population at Pueblo Community Health Center Inc. The Behavioral Health Care Coordinator manages and directs in coming phone calls and clinic BH triage calls. Generates and distributes various reports and schedules patients for BH therapy and for psychiatric treatment. BH Care Coordinator facilitates behavioral health release of information requests to and from PCHC. Manage BH provider schedules and may interact with patients who may be experiencing an emotional or mental crisis

Education/Experience/License/Certification:


  • Bachelor’s Degree with three years relevant experience in community primary health care required. An equivalent combination of education and experience may be considered
  • Work experience demonstrating strong ability in the following areas: leadership, advocacy, communication skills both written and verbal, resource research, creative problem solving and sensitivity/responsiveness to varying cultural characteristics and beliefs.
  • Valid driver’s license and auto insurance as required by law.

  • Occasional travel may be required including day-to-day local travel.

    • Ensure the function and activities of this department to embrace the philosophy, mission, values and Communicate with Heart service model supported by the Board of Directors of Pueblo Community Health Center, Inc.
    • Adhere to the guidelines and procedures of Pueblo Community Health Center, Inc.
    • Facilitate Patient access to Pueblo Community Health Center, staff and resources at Pueblo Community Health Center.
    • Facilitate access to behavioral health services for the patient population at Pueblo Community Health Center Inc.
    • Manage and direct in coming phone calls for the BH team
    • Schedule patients for BH therapy and for psychiatric treatment.
    • Facilitate behavioral health release of information requests to and from PCHC
    • Manage BH provider schedules to assure schedules are filled per protocol and productivity standards.
    • May interact with patients who may be experiencing an emotional or mental crisis and assures warm hand off to appropriate BH provider.
    • Generate and distribute various reports as assigned by Director.
    • Document patient contacts and interaction in the medical record.
    • Develop and maintain a professional relationship with patients, medical and behavioral health providers of Pueblo Community Health Center.
    • Follow up with patients on missed BH appointments.
    • Serve as a contact person, advocate and informational resource for the patients, families, PCHC providers and community stakeholders.
    • Educate, support, and provide appropriate referrals to patients and families to facilitate access to and appropriate utilization of resources.
    • Provide timely communication, inquiry, follow up and integration of information into the medical record.
    • Documents in EHR the assignment of BH patient panels.
    • Maintain patient confidentiality.
    • Perform other related duties as assigned.
