The Behavioral Health Home team at Woodfords Family Services is searching for qualified BHH Coordinators to join their growing team of professionals!

This is a full-time position that requires frequent travel in and around York and Cumberland Counties.

Woodfords Family Services is a nonprofit organization that's been committed to the support and inclusion of people with special needs and their families in Maine communities for over 55 years! Today, the organization provides an array of clinical, behavioral health and educational programs to more than 2,000 children, youth and adults living with autism, developmental disabilities, intellectual disabilities and/or mental health diagnoses across the state of Maine.


Position Summary

As a BHH Coordinator, you will be an integral part of a team that's responsible for working with children with disabilities and their families to provide support, advocacy, and connection to services that encourage and promote family empowerment and self-sufficiency. This will be achieved through a variety of channels including: Assessment and evaluation, plan of care development, maintenance and review, and facilitating and attending family/team meetings.

Responsibilities:

  • Meets regularly with child and family in a variety of settings to assess consumer functioning, progress, quality of life and health and safety issues, and to address any concerns identified.
  • Possess knowledge regarding community resources and assists families with referrals to such resources.
  • Conducts strength and needs assessments for all children and facilities family wrap around meetings as needed.
  • Serves as a liaison with other agencies involved in providing other types of services to the child (public schools, behavioral health agencies, consultants, etc.) to ensure appropriate level of care and where deficient, direct additional resources.

Qualifications:

  • Must possess a Bachelors Degree in Social Work or related Human Service field (LSW Preferred).
  • Must possess at least (3) three years experience in the field.
  • Must be knowledgeable about regulatory guidelines
  • Must possess a valid State of Maine driver's license and have use of a vehicle.
