  • Position Summary: This position supports the mission of The Medical Center of Aurora optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Behavioral Health Case Manager is responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care. Balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent clinical case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues and education needs will also be monitored for and reported to the QRM DEPARTMENT as a part of the CQI process. When need arises, will perform other duties as assigned by supervisor.

  • Position Requirements:

  • Licensure/Certification/Registration: LCSW, LPC, LMFT (Preferred). BLS - Healthcare provider through the American Heart Association or American Red Cross (required). Special certification in case management preferred.

  • Education: BSN or Master Degree in Mental Health Related Profession (preferred).

  • Experience: 5 or more year's clinical experience required. 1 to 3 years Case Management experience preferred with emphasis on utilization review (Behavioral Health Utilization Review preferred).
  • Special Qualifications: Ability to assess information, apply critical thinking processes to determine course of action, define expected outcomes and evaluate effectiveness of plan through continuous assessment of the patients progress. Ability to compile, evaluate and report statistics to members of the team and utilize the information to facilitate process improvement activities. Strong oral/written communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Requires excellent human relations skills and strong analytical/organizational skills. Must have ability to perform multiple tasks simultaneously and work in high stress environment. Position requires flexibility and cross coverage.

SKILLS:

Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention
