SUMMARY
Under the supervision of the Substance Abuse Therapist & Administrative Supervisor, the Behavioral Health Case Manger works in the medical clinic and arranges services for individuals with co-occurring and/or medical concerns. Services include: screenings, coordination of care, referrals, case consultations with medical providers and advocacy. The Behavioral Health Case Manager also provides brief supportive services, crisis intervention and substance use/mental health related education for patients, guardians and their families.

ESSENTIAL JOB FUNCTIONS including the following. Other duties may be assigned:
Maintains a courteous, helpful and contentious attitude in dealing with coworkers and patients and their families ensuring professionalism and confidentiality at all times.
Provides education and encouragement to patient, guardian and support system.
Provides crisis and behavioral health intervention and assists providers as necessary.
Collaborates with physicians, health care team members and patient/families to coordinate care.
Acts as a resource specialist to patients seeing their medical providers.
Assists with needs based of Social Determinates of Health Screenings.
Provide appropriate follow up as needed.
Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
Assess and address motivational and psycho-social issues and provide appropriate referrals if needed.
Completes appropriate documentation necessary through routine observation.
Regular and prompt attendance at work.
Demonstrates flexible and efficient time management and ability to prioritize workload.
All other duties as assigned.

QUALIFICATIONS

KNOWLEDGE/SKILL/ABILITY
Strong written and verbal communication skills
Excellent interpersonal skills to relate effectively with co-workers and patients
Ability to problem solve and de-escalate crisis situations
Experience with Individual, family, and group therapy practice
Ability to work a flexible schedule
Working knowledge of clinical diagnoses for mental health population
Computer literacy and ability to conduct data entry

EDUCATION, CERTIFICATES, LICENSES, AND/OR EXPERIENCE
Possess State of Michigan licensure as an LMSW, LLMSW, LBSW, LLBSW, LPC or other relevant social service degrees or State of Michigan licensure as a Registered Nurse, or State of Michigan licensure as a Licensed Practical Nurse. Experience in the use of and training of Evidence Based Clinical Practices, preferred. One (1) year experience working with patients with mental health disorders, Substance use experience is preferred.
