Job description

Job Summary: The Case Manager will develop and/or deliver ongoing school and community based assessment, treatment and support for children and youth experiencing a severe emotional disturbance and their families. The Case Manager will provide direction to members of the child’s treatment team, facilitate team meetings and provide service coordination, treatment planning, supervision and crisis intervention. The Case Manager will also be a part of the leadership team that provides training to other staff members.

Special Requirements · Prior experience with public school based intervention program preferred. · Superior interpersonal skills and the ability to function in a team required. · Bachelor’s Degree in human services or related field required. Two years of human service delivery with children and families is preferred.
