Description:Lutheran Life Communities’ story began in 1892 with 26 cents. In the late 1880s, Pastor August Reinke preached a sermon to parishioners at Chicago’s Bethlehem Lutheran Church. He focused on the obligation to care for the community’s aging population. Two days later, an envelope appeared on his desk. It read “for the Altenheim building fund.” In the envelope, Rev. Reinke found 26 cents. Gifts continued and, in 1892 an organization was formed.

The CNA Ambassador will provide companionship and assist with the daily personal needs of Hearthstone residents in order to provide a safe, quality environment, and lifestyle.
Requirements:Primary Responsibilities
- Follows established plan of care for each individual resident/client to maintain the resident’s highest level of independent living. Also communicates resident issues to next shift to ensure consistent, quality care.
- Follows procedures for fire drills and other emergencies.
- Offers courteous reminder to resident to self-administer medications on correct day and time from appropriate container to ensure medication compliance. Documents med reminder sheet, immediately reporting any concerns to supervisor or appropriate contact.
- Responsible for all other duties assigned within the community which they are working in, including assistance in tray delivery, social activities and clerical work.
- Performs duties and carries out responsibilities in a professional, caring, courteous manner in accordance with the policies and procedures of Hearthstone. Works cooperatively with the residents, families and staff.
- Helps residents in a variety of tasks related to personal care such as assisting with bathing, dressing, shaving, shampooing hair, and oral hygiene as appropriate.
- Transports, escorts and accompanies residents to dining room/common areas, shopping trips, errands and various personal appointments, as needed. Provides assistance with ambulation & repositioning when necessary.
- To ensure resident’s safety, looks for unsafe conditions in resident’s apartment, immediately reporting any unsafe condition observed; as well as throughout the Hearthstone environment to supervisor.
- Follows and completes required/requested schedule of service in a timely, accurate, and efficient manner. Clearly and concisely reports issues to supervisor in a timely basis – verbally and in writing.

- Support the program elements by daily implementation and participation of functions (i.e., huddles) and demonstration of the Foundational Documents.

Candidate Requirements
- Previous experience with older adults.
- State of Illinois certified nursing assistant (C.N.A.) certification is required.
- Must be able to read and write legibly in English.
- Must be able to comprehend and perform oral and written instructions and procedures.
- Must be able to verbally communicate information to customers and other employees.
- A current CPR certification, preferred.
- Work schedule may include working weekends, holidays and all shifts.

Physical Requirements & Work Environment
- While performing the duties of this job, the employee is subject to walking and standing for prolonged periods: and frequently stoops, kneels, crouches, or crawls, pushes, pulls, grasps and feels.
- The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with residents and handling equipment.
- The employee is subject to assist and support up to 50 pounds with varying amounts of assistance on a reoccurring basis.
- The employee is required to have corrected vision and hearing within normal range.
- The noise level is normally quiet.
- Occupational exposure to blood, body fluids, infectious substances, including communicable diseases when performing resident care (Category 1).
- Occasional exposure to cleaning agents and disinfectants.
- Environment is home-like.
