Job Title: AI-Life Health Navigator
Job Summary:
The Health Navigator is responsible for implementing a client-centered service delivery model aimed at reducing barriers to care across the health care continuum. The Health Navigator is a key position in Linkage and Navigation program working towards identifying client’s needs and will facilitate access to medical, social and other prevention and essential supportive services for all high-risk HIV-negative and HIV-positive clients who are MSM ages 30+ and help clients move throughout the system of medical and social services. The Health Navigator will serve as the primary point of contact for clients while providing education, appointment scheduling support, follow-ups, case management and benefits support.
Primary Duties & Responsibilities include, but are not limited to:
· Provide affirming and up-to-date education on topics that include, but not limited to condoms and safer sex products, PreP, Emergency PEP, iART, HIV care and treatment, U=U, HIV/STI screening, reproductive health, mental health, substance use and supportive services
· Utilize a client-centered, harm-reduction framework service model to determine needs and develop a client-centered service delivery plan that support the client in making their own autonomous choices about their health care, well-being, and sexual health needs
· Connect and accompany clients when needed to medical, social services and other prevention and essential supportive services
· Assist clients with initial and ongoing scheduling, tracking, sending reminders for, and rescheduling (as necessary) their appointments for services either onsite, virtually, or at partner agencies for which the client is linked
· Conduct appointment support via in-person, email. SMS, Social media, or other internet-based methods and/or phone calls
· Complete follow-up activities to facilitate continued client engagement to discuss medication adherence, identifying and addressing additional service needs, preparing a client for upcoming services, conducting post-appointment support and verification
· Offer case management services to clients in need of more extensive coordination of care
· Complete intakes, assessments, service plans and referrals to essential support services
· Conduct home visits as needed to monitor services and client status.
· Train in Anti-Retroviral Treatment and Access to Services (ARTAS) and Health Navigation Services (HNS) strengths-based case management model
· Provide benefits navigation services such as insurance and patient assistance programs for PrEP, emergency PEP, iART, HIV primary care, primary health care, mental health, substance use and other supportive services
· Educate and support clients navigate their insurance plans member services
· Engage in case finding activities and outreach to community organizations, churches, food pantry, community partners, internal and external agency programs
· Maintain a monthly caseload of 40 unique clients
· Document all elements of service encounters in charts and in required database noting service needs, progress towards viral suppression and access to other essential support services and biomedical interventions
· Attend training seminars, case conferences, staff meetings and data driven supervision
· Conduct outreach and recruitment activities at medical/non-medical and community service providers to engage clients in linkage readiness and program enrollment
· Obtain primary care service measures for all enrolled clients and work to ensure clients are retained in care to achieve viral load suppression
· Offer/Provide condoms to all engaged in Health Navigation Services
· Other related duties as assigned.
Qualifications/Criteria:
· Minimum of 1 year experience (paid, peer or volunteer in HIV/HCV education or counseling).
· Extensive knowledge of HIV/HCV/STI infection, transmission, prevention;
· Knowledge of HIV/AIDS, sensitivity towards PLWHAs.
· Experience working with high risk populations such as injection drug users.
· Knowledge of High Impact Prevention strategies and evidence based behavioral interventions
· Knowledge of community resources.
· Excellent verbal and written communication skills, including strong interpersonal and organizational skills.
