POSITION TITLE: After Care Social Worker

PROGRAM: Qualified Residential Treatment Program (QRTP) & Prenatal, Post-Partum, Parenting Program (PPP)

DEPARTMENT: Family Permanency Services

REPORTS TO: Director of Residential Treatment Services Program

Description Catholic Guardian Services foster care residential program is licensed by the NYS Office of Children and Family Services (OCFS) as a Qualified Residential Treatment Program (QRTP) and our mother-child group home is licensed as an exempt QRTP as it is certified as a Prenatal, Post-Partum, and Parenting Program. Our foster care residential program provides services and support to youth struggling with psychological, behavioral, and/or substance use issues. Our interdisciplinary program team provides general supervision, coordination of medical services, on-site mental health treatment, structured recreational activities, emotional support, and services around education, vocational training, and employment development.

Our program creates a therapeutic milieu through implementation of Dialectical Behavioral Therapy (DBT) Skills which addresses the complex needs of youth placed in the program. The program also helps adolescents and their families to establish improved relationships, and whenever possible, reunification. For adolescents returning home, staff helps to establish the long-term community supports needed for a successful life upon discharge. For older youths leaving care, help is provided to strengthen connections to family and ensure economic stability and social supports for a successful transition to adulthood.

Major Duties & Responsibilities: As an OCFS certified Qualified Residential Treatment Program (QRTP) and Prenatal, PostPartum, Parenting Program (PPP) we must provide discharge planning and family-based aftercare support for at least six months post-discharge from the QRTP or PPP. The After Care Social worker is responsible for: Creating a measurable discharge plan with the youth, his/her family or discharge resource and the program?s permanency team. The goals in a youths discharge plan must be in alignment with the youth?s QRTP treatment plan
