Summary: The ACT Team Housing Case Manager is responsible for providing community based support and training treatment to help mentally ill adult clients obtain the necessities of daily living.

Responsibilities:

  • Make home visits to clients to provide practical help and supports, advocacy, coordination, side by side individual support, problem solving, direct assistance, training and supervision to help client obtain the necessities of daily living including medical and health care, legal and advocacy service, financial support such as entitlement, housing, money management services and transportation.
  • Assist clients in obtaining and maintaining appropriate housing that meets individual needs and preference. Refers clients to appropriate resources and services as needed.
  • Assist and support client to perform household activities.
  • Provide ongoing assessment, problem solving, side by side services, skill training, supervision and environmental adaptions to client with activities of daily skills.
  • Assist and support clients to carry out hygiene and grooming tasks.
  • Provide nutrition education and assistance with meal planning, grocery shopping, and food Preparation.
  • Provide side and side and coaching to help clients socialize and locate leisure activities.
  • Provides service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team, assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plan in coordination with client, treatment team, ensuring immediate changes are in the treatment plan as clients' needs changes, educating, supporting, clients' families and advocating for clients' rights and preferences.

Requires:

  • Bachelor's degree from accredited college or university.
  • Valid Maryland driver's license with 3 or fewer points.
  • Possesses own vehicle and provides proof of auto liability insurance.
  • Possesses knowledge of the types of mental illness and their symptoms.
  • Demonstrates excellent interpersonal skills and ability to work effectively with clients, care-givers and other professionals who work effectively with clients, care-givers and other professionals who work with the client.
  • Knowledgeable of community resources and referral processes.
  • Able to use the personnel computer to maintain required documentation.
