FT: (.9-1.0) $2000 SIGN ON BONUS
PT: (.5-.8) $1200 SIGN ON BONUS

1st Shift: 6:30am-2:30pm OR 6:30am-10:30am *Part-time Available

2nd Shift: 2:30pm -10:30pm *Full/Part-time Available

Congregational Home is a non-profit; faith based continuing care retirement community. At Congregational Home, we provide a full continuum of care for seniors. We offer rehabilitation, assisted living, skilled nursing and memory care services. We are looking for individuals that can adapt to the needs of residents in a variety of areas.

Job Responsibilities:

Under the supervision and direction of a licensed nurse, responsibilities of this position include, but are not limited to:

• · Performs activities of daily living for residents such as bathing, oral care, grooming, dressing, toileting, feeding, walking, exercising and encouraging independence
• · Observes patients' conditions, measuring and recording food and liquid intake and output and vital signs
• · Transfers residents via wheelchairs and performs patient transfers in accordance with plan of care (i.e. sit to stand and hoyers)
• · Passes medications, if able to provide documentation of completed medication training

Qualifications:

• previous experience in a long-term care facility preferred, but not required
• must be able to express self in written and oral communication and communicate effectively in care setting with residents, families and staff members
• ability to maintain confidentiality
• ability to work independently or with a team
• conducts oneself in a caring, sensitive and safe manner which promotes positive relations and rapport with residents and visitors

Educational Requirements: must hold a current Certified Nursing Aide certification in the state of Wisconsin
